Pancreatitis is a complex, progressively destructive inflammatory disorder. Alcohol was long thought to be the primary causative agent, but genetic contributions have been of interest since the discovery that rare PRSS1, CFTR and SPINK1 variants were associated with pancreatitis risk. We now report two associations at genome-wide significance identified and replicated at PRSS1-PRSS2 and X-linked CLDN2 through a two-stage genome-wide study. The PRSS1 variant likely affects disease susceptibility by altering expression of the primary trypsinogen gene. The CLDN2 risk allele is (...) associated with atypical localization of claudin-2 in pancreatic acinar cells. The homozygous CLDN2 genotype confers the greatest risk, and its alleles interact with alcohol consumption to amplify risk. Cytokines are polypeptides released by activated vertebrate blood cells which have profound effects on other blood cells and which have hormone‐like properties affecting other organ systems as well. In recent years a wide variety of these mediators has been isolated and characterized. Many of these molecules have subsequently been cloned and expressed in E. coli. The tremendous importance of these proteins to host immune and non‐specific defense systems along with the striking similarities of their properties among different species suggested to (...) us that cytokines may have been proteins that have been conserved through evolution. Investigations of the evolution of cytokines will help us decipher the complex cellular, humoral and molecular interactions that regulate host defenses. Studies of the invertebrates will shed light on the phylogenetic emergence of these molecules as well. (shrink)

The sciences are characterized by what is sometimes called a “methodological naturalism,” which disregards talk of divine agency. In response to those who argue that this reflects a dogmatic materialism, a number of philosophers have offered a pragmatic defense. The naturalism of the sciences, they argue, is provisional and defeasible: it is justified by the fact that unsuccessful theistic explanations have been superseded by successful natural ones. But this defense is inconsistent with the history of the sciences. The sciences have (...) always exhibited what we call a domain naturalism. They have never invoked divine agency, but have always focused on the causal structure of the natural world. It is not the case, therefore, that the sciences once employed theistic explanations and then abandoned them. The naturalism of the sciences is as old as science itself. (shrink)

Fluctuations in endogenous opioid activity in the brain, controlled under ordinary conditions by attachment, are capable of producing patterns of dependence in social behavior resembling those appearing in substance abusers. Withdrawal symptoms arising in relation to these fluctuations, short of producing dependence, ordinarily fuel everyday social interaction, and interaction then serves to modulate opioid activity within a range associated with comfort. Comfort-constraints in this sense operate in all settings of social interaction, part of an innate caregiving mechanism conserved by evolution (...) in human behavior. In this paper we present a formal model of the neurosociological mechanism embodying these comfort constraints. Conceptualized as a hyperstructure, the mechanism grounds thinking about social interaction in recent biological discoveries about the brain, and enables sociologists to study how activity in core brain systems constrains deep patterns in social life, including the human tendencies to altruism and reciprocity. Using computational methods, we undertake simulations to study the mechanism, deriving implications about moral behavior. The theory of the hyper-structure leads to new conclusions about reciprocity and altruism, and bears upon sociological understanding of related subjects such as justice and social comparison. Brain damage may doubly dissociate cognitive modules, but the practice of revealing dissociations is predicated on modularity being true (T. Shallice, 1988). This article questions the utility of assuming modularity, as it examines a paradigmatic double dissociation of reading modules. Reading modules illustrate two general problems. First, modularity fails to converge on a fixed set of exclusionary criteria that define pure cases. As a consequence, competing modular theories force perennial quests for purer cases, which simply perpetuates growth in the list (...) of exclusionary criteria. The first problem leads, in part, to the second problem. Modularity fails to converge on a fixed set of pure cases. The second failure perpetuates unending fractionation into more modules. (shrink)

Among the most influential and enigmatic thinkers of the modern age, Nietzsche and Heidegger have become pivotal in the struggle to define postmodernism. In this work, Gregory Smith offers the most comprehensive examination to date of the turn to postmodernity in the writings of these philosophers. Smith argues that, while much of postmodern thought is rooted in Nietzsche and Heidegger, it has ironically attempted, whether unwittingly or by design, to deflect their philosophy back onto a modern path. (...) Other alternative paths emanating from both Nietzschean and Heideggerian thought that might more powerfully speak to postmodern culture have been ignored. Nietzsche and Heidegger, Smith suggests, have made possible a far more revolutionary critique of modernity then even their most ardent postmodern admirers have realized. Smith contends that the influences on the postmodern in the thought of Nietzsche and Heidegger are founded in a new vision of praxis liberated from theory. Ultimately, these philosophers do transcend the nihilism often found in the guise of postmodernism. Their thought is, moreover, consistent with the possibility of limited constitutional government and the rule of law. Smith's book takes the first step toward recovering these possibilities and posing the fundamental questions of politics and ethics in ways that have heretofore been closed off by late-modern thought. The understanding of emergent, self-organizing phenomena has been immensely deepened in recent years on the basis of simulation-based theoretical research. We discuss these new ideas, and illustrate them using examples from several fields. Our discussion serves to introduce equivalent self-organized phenomena in social interaction. Interaction systems appear to be structured partly by virtue of such emergents. These appear under specific conditions: When cognitive buffering is inadequate relative to the levels of stress persons are subjected to, anxiety-spreading has the potential of (...) pushing their interaction into nonlinear conditions. Arousal in these conditions produces effects on behavior arising from biological sources-indeed, behavior can come under the control of reflex patterns. When this occurs, psychological activity no longer screens off biological controls over behavior. As the direct effects of biological activity spill into interaction, attachment behavior introduced into an interaction system can produce effects that are transmitted beyond dyads to produce global social patterns. These effects illustrate how strong interactions based in biological activity can produce an architecture for social systems. (shrink)

In contemporary bioethics, a live debate exists about how to properly adjudicate the conscientious objections of physicians and other health professionals to certain practices—such as abortion and euthanasia—that are deemed medical in some jurisdictions. However, surprisingly, this debate often takes place without a clear definition of terms. This article will contribute to this debate by orienting the question of conscientious objection in direct relation to conscience, law, and the common good which unites and directs multiple parties toward a common end, (...) using the lens of Thomistic philosophy. Building on this foundation, it will suggest a framework for evaluating instances of conscientious objection in medicine. (shrink)

Erving Goffman's distinctive contribution to an understanding of others was grounded in his information control and ritual models of the interaction process. This contribution centered on the forms of the interaction order rather than self-other relations as traditionally conceived in phenomenology. Goffman came to phenomenology as a sympathetic but critical outsider who sought resources for the sociological mining of the interaction order. His engagement with phenomenological thinkers (principally Gustav Ichheiser, Jean-Paul Sartre and Alfred Schutz) has to be understood in these (...) terms. The article traces basic differences in analytical focus through a range of phenomenological critiques of Goffman and a comparison of salient aspects of Schutz's and Goffman's writings. While the contrasts have perhaps been overplayed, I conclude that Goffman's thinking about others probably owed more to his pragmatist roots than to his later encounters with phenomenology. (shrink)

The function of REM, or any other stage of sleep, can currently only be conjectured. A rational evaluation of the role of REM in memory processing requires systematic testing of hypotheses that are optimally derived from a complete synthesis of existing knowledge. Our view is that the large number of studies supporting a relationship between REM-related brain activity and memory is not easily explained away. [Vertes & Eastman].

In Black Museum, a young woman is out to take revenge on the man who imprisoned her father's digital self in a museum exhibit that allows sadistic visitors to reenact his execution. While the exhibit is morally detestable and some may think that the museum's curator gets what he deserves in the end, the woman's act of vengeance is morally disturbing. This chapter explores Martha Nussbaum's account of anger and forgiveness and considers Christian and Buddhist teachings. An argument by David (...) Kyle Johnson for the moral permissibility of revenge under certain circumstances is considered and rejected. The authors conclude that the act of revenge in Black Museum is not morally justified. Cardiopulmonary resuscitation of a patient with an uncertain resuscitation status, and a discharging implantable cardiac defibrillator, presents a significant ethical challenge to healthcare professionals in the emergency department. Presently, no literature discusses these challenges or their implications for ethical healthcare delivery. This report will discuss the issues that arose during the management of such a case and attempt to raise awareness among healthcare professionals to ensure better preparation for similar situations.

Recent research on consumer social responsibility highlights the need to examine psychological drivers of environmentally‐friendly consumption choices in a global context. This article investigates consumers’ willingness to pay (WTP) more for environmentally‐friendly products across 28 European Union (EU) countries, using a sample of 21,514 consumers. A multigroup structural equation modeling analysis reveals significantly different patterns and relationships, in how (a) subjective knowledge about the product's environmental impact, (b) environmental product attitudes, and (c) the perceived importance of the products’ environmental impact (...) influence consumers’ WTP more for environmentally‐friendly products across countries. The hypothesized model predicts WTP for 20 out of 28 countries and the findings show that a “one‐size‐fits‐all” approach is inadequate in capturing the heterogeneity of EU consumers. Hosfstede's cultural dimensions of uncertainty tolerance and individualism explain differences in WTP for environmentally‐friendly products across EU countries. Elizabeth Anderson claims that the prevailing culture of business is one of domination. “Most workplace governments in the United States are dictatorships, in which bosses.. don’t merely govern workers; they dominate them” (2017, p. xxii; italics in the original). If this diagnosis is correct, then the culture of business poses a significant threat to human liberty, as each year millions of people in the employ of businesses spend hundreds or thousands of hours on the job. This essay provides a further (...) argument supporting Anderson’s analysis, by extending her treatment of Adam Smith and drawing on his claim about the potentially mind-numbing effects on workers of extreme division of labor. Smith’s analysis, though consistent with Anderson’s, implies that the problem is more worrisome than she allows, and accordingly that Anderson’s own remedy might be insufficient. Our Anderson/Smith argument suggests that worker unfreedom might warrant more aggressive institutional remedy. (shrink)

Gregory T. Doolan provides here the first detailed consideration of the divine ideas as causal principles. He examines Thomas Aquinas's philosophical doctrine of the divine ideas and convincingly argues that it is an essential element of his metaphysics. According to Thomas, the ideas in the mind of God are not only principles of his knowledge, but they are productive principles as well. In this role, God's ideas act as exemplars for things that he creates. As Doolan shows, this theory (...) of exemplarism is an integral part of Thomas's account of the existence and order of the created universe. It also accounts in part for the freedom of God's creative act. The volume begins with an introduction to Thomas's doctrine of exemplarism and then addresses his arguments for the existence of exemplar ideas within the mind of God. Having established the existence of divine ideas, Doolan considers how Thomas reconciles their multiplicity with God's simplicity. The work identifies the various things for which Thomas posits divine ideas. As Doolan is careful to show, Thomas does not consider all of these ideas to be exemplars because not all of them act as causal principles. After identifying the ideas that do act in this way, Doolan considers how such exemplars can be causes of natural things without compromising the causality of natural agents. The volume culminates with a consideration of the role that the divine ideas play within the structure of Thomas's theory of participation. ABOUT THE AUTHOR: Gregory T. In lieu of an abstract, here is a brief excerpt of the content:On PartnershipRyan Schwarz, Duncan Smith-Rohrberg Maru, Dan Schwarz, Bibhav Acharya, Bijay Acharya, Ruma Rajbhandari, Jason Andrews, Gregory Karelas, Ranju Sharma, and Mark ArnoldyRecently, Bayalpata Hospital, in the rural district of Achham, Nepal almost collapsed under the weight of its own staff's discontent. The hospital had been largely abandoned until 2009 when our organization, Nyaya Health, renovated and opened it in partnership with the Nepali government. Since then, (...) the hospital has seen great progress and has experienced widespread community support. Nonetheless, earlier this year, a broad-based staff revolt occurred, led primarily by our senior clinical staff members, raising concerns that Nyaya's management policies were ineffective, and wages and benefits were too low. The unrest included everything from a strike, limiting services to our patients, to aggression against non-strikers and slander and racist comments in public media against the Nyaya Health Board of Directors. Ultimately, the conflict was resolved, though not without the departure of our three most senior clinicians. The incident deeply damaged staff morale [End Page 101] and impeded basic services to our patients, and from our perspective as a leadership and management team, it was deeply troubling and spiritually challenging. We have learned much in the wake of the strike, but here we focus more narrowly on the concept of partnerships in our work by examining the recent strike and the two key issues that lead to it: wages and benefits, and management policies at Bayalpata Hospital.In grappling with the recent staff crisis, some of our leadership team expressed concerns that we would never be able to develop a true partnership in Achham if our senior-level employees led strikes against the organization. The leaders of the strike were our most well-educated and high-ranking staff. They were also individuals who had come to work for us specifically citing the moral imperative of our work, and their pride in helping to serve the poorest of the poor. One leadership team member asked, "What is 'partnership' then? We came into this work with the premise that we would work together, with our partnership built around the right to health—but who are our partners if they lead strikes damaging the very services our collective mission aims to offer?"Another leadership team member commented, "The first time I visited Achham I was invited for tea at Meena's* house (one of Nyaya's midwives). We arrived at her house and Meena greeted us warmly. She showed us in to a small room: there was no furniture, but there was an immaculately swept mat which we joined her on. We drank tea, seated on her rug, and listened to her tell us how she had come to live in the house, how she had come to work for Nyaya, and her pride in now being able to provide both for her own children, as well as, the children of her relatives. As the only employed family member, she now had the challenge of providing for all of them. When we question our partnership, I'm taken back to Meena's warm, but extremely poor, home. She has worked for Nyaya for almost four years. She has helped our organization provide free care to over 75,000 people. Is this not a type of 'partnership'?"Allow us to offer some background on the issue of wages—Meena has been with our team since 2008, and has been paid about $2,500 per year—a generous wage compared to other employers in the region. Meena was one of the striking staff members. Among other concerns about the way the hospital was being run, she and other staff asked indignantly why we couldn't provide them with higher wages, better benefits, and more resources, both for her and the hospital. Notably, Meena's salary (and the rest of our staff's) was well above her equivalent in other, more urban hospitals (to compensate for the extremely remote area in which we work), and in a district where most families are farmers, our staff's salaries occupy the highest income bracket in the region.
